How do I request medical information from rediclinic?

The Patient Portal is an Internet service that allows our patients to: communicate with us; access their health records; request or reschedule appointments; view and update personal information; request prescriptions; receive test results; read patient educational material; view statements; pay bills; and access related services and content.

When will I receive a response to my Patient Portal request?

(1) To receive a complete copy of your RediClinic medical records; or (2) To have a copy of your RediClinic medical records sent to a health practitioner of your choice, click here to access a “Medical Request Form”, completely fill out the form, and send either of the following ways: (a) To the address listed on the form; or

What insurance companies does rediclinic accept?

Our staff will contact you sometime between Monday and Friday from 8AM to 5PM. Please note the patient portal is not monitored 24 hours a day, 7 days a week; we will respond as soon as possible but expect a minimum of 2-3 business days . If you have a more urgent matter please call the office so we can assist you on any urgent matter. Thank you!

Life at Eating Recovery Center Milwaukee

Patients and Families at ERC Milwaukee will work closely with a compassionate team of providers including board certified physicians/psychiatrists, registered nurses, licensed therapists, registered dietitians and additional experts in eating disorders.

Family and Alumni Support

Our support for patients and families does not end at the time of discharge—rather, the treatment team carefully develops a personalized discharge plan, and offers several aftercare services and alumni support programs for patients that do not have an established eating disorder treatment team.

Travel

For our out-of-town patients and families, we offer free Travel Concierge Services.

Child and Adolescent Treatment

Eating Recovery Center's specialized programming for children and adolescents of all genders allows even younger patients receive the eating disorder care they need in a comfortable, safe and compassionate environment.

Paying for Treatment

Insurances accepted change frequently. If you do not see your insurance carrier here, please ask us. Our insurance and financial specialists will provide a free benefits check.
