Radiology Associates offers appointment requests online for most exams.#N#Please Click on “Request An Appointment” button below then#N#select the exam you are requesting and fill out the form.#N#Our Patient Services Coordinator will contact you by phone with a day and time for your appointment.

To ensure accurate scheduling and excellent patient care, we require a copy of your order form you received from your doctor. You may take a picture via your smartphone and email it to orders@ra-slo.com
