Our patient portal is a tool that will allow you to view your own medical records and communicate with our office and your provider’s staff quickly and in a secure manner.

Patient Registration – Phreesia

Proliance Puget Sound Orthopaedics (PSO) utilizes a secure, online patient appointment portal called Phreesia. The Phreesia portal allows you to fill out your health information and medical history prior to your appointment or procedure.

Insurance and Authorizations

Proliance Puget Sound Orthopaedics participates in most insurance plans, Medicare on assignment, and most Labor and Industries and Workers Compensation claims.

Preparing for Visit

Your first appointment will begin with going over your medical history pre-registration form and conducting a physical examination. The history gives your physician an idea about the type of symptoms or pain you may be experiencing, your past medical and surgical history and medications that you are currently taking.

INSURANCE AUTHORIZATIONS

Special testing includes MRI, bone scan, doppler studies, joint and spine injections, CT/CAT scans, DEXA scans, ultrasound, and EMG. Insurance processing times vary depending on your insurance. If you have not been contacted within the estimated time frame, then contact your scheduler. Review our Special Testing Notification for more information.
