Proxy Access - A parent or legal guardian of a minor child up to the child’s 12th birthday or individuals age 18 or older may request proxy access to another individual’s Parkview Patient Portal account by completing the Proxy Access portion of the Parkview Patient Portal Consent Form and submitting it to Parkview Community Hospital Medical Center.
