Your activation code will be printed on your after-visit summary, can be printed for you or sent to the email address we have on file for you, upon your request. You will not need to use this code after you have completed the sign-up process.
Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to login and create your own user ID and password. If you were not issued an activation code, you may call your primary care clinic to get one or ask to sign up during you next office visit.

If you decline consent to share your information with affiliated partners, you will have multiple MyChart accounts: one for each organization, each with its own username and password. You will need to sign in to the specific MyChart account in order to view your information for that organization.

To get to your saved payment information, click Set Up Auto Pay. Hover your mouse over the saved payment information and the Delete button will appear. Click Delete to remove all your saved payment information.
