Is health care a one size fits all service?

Health care isn’t a one-size-fits-all service. Some circumstances call for a more specialized professional will a different skill set. For these patients in need of specialty care, we offer cardiology, dermatology and gastroenterology services.

COVID-19 Vaccine Scheduling

Getting a COVID-19 vaccine is one step you can take to keep you and your family safe. There are now booster recommendations for all three available COVID-19 vaccines. Eligible individuals may choose which vaccine they receive as a booster dose.

Does Ohio have a PE program?

Ohio has granted permission for community health centers to participate in Presumptive Eligibility (PE) Program. The PE program allows us to provide immediate access to health services by giving our patients and their families, temporary health coverage through Medicaid if they are presumed to be eligible by us under the current Ohio Medicaid guidelines. To apply for temporary and ongoing Medicaid you will need to bring: 1 month of household income (current within the last 30 days), social security numbers for all family members requesting assistance, and identification for all family members requesting assistance.

Do uninsured patients get a sliding fee discount?

We offer a sliding fee discount for our uninsured patients based on a patient’s household income and family size. To apply for the sliding fee discount patients will need to provide proof of income for all members of the household (see below for acceptable income). Sliding fee charges will be determined once income documentation has been obtained and calculated.

The Primary Health Network Patient Portal is for non-urgent issues only. If you are experiencing an urgent medical issue or need a same-day appointment, please call your clinic. If it is an emergency, dial 911.

What happens if my patient portal is locked?

As a general security feature, the patient portal will lock-out any account that has had (3) failed login attempts. If your account has been locked, you must visit your physician's office in order to verify your identity in person. Please bring valid photo identification with you ( i.e. driver's license ). We apologize in advance for this inconvenience, but it is necessary for security purposes.

Is the Patient Portal for non-emergent issues?

No, the Patient Portal is intended for non-emergent healthcare issues only. Please contact your physician's office for questions that you would like answered within three business days.

Is the Patient Portal secure?

Your personal information is extremely secure. The information on the Patient Portal is merely a snapshot of your medical record, not the actual record. Portal records are maintained on a secure server.
