We require the following information to set up for portal: – Name – Date of Birth – Phone – Email – Mailing Address Once your portal is created, you will receive an email.

After growing up in the panhandle of Florida, Heather received her undergraduate degree in Nursing at Florida State University and then earned her Master’s degree in Nursing at the University of Florida. While earning her master’s degree, Heather worked at Florida Hospital in their critical care department.
