APPOINTMENTS and MEDICAL RECORDS

Your current doctor will not be able to see you after July 31, 2021, unless you have joined their new practice as a member. Appointments after August 1 will be confirmed by their new practice.

Patient Portal

With the dissolution of PPA the Intelichart Patient Portal will also go away on or about August 1. In accordance with HIPPA privacy laws, all Personal Health Information (PHI) will be wiped clear of the portal by Intelichart. You are encouraged to download any information to your local computer device or drive in advance of the Portal shut down.
