All patient medical records have been securely transferred over to the new system for your convenience. In order to actively use the new patient portal (Elation Passport) an invitation must be sent from your clinic staff. Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Northwest Health - Porter will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.
