Step 2: Call to Make an Appointment

We are currently accepting new patients! To book an appointment, please call our office and our team will help schedule an appointment with the appropriate practitioner at a convenient time.

Step 3: Complete New Patient Packet

All New Patients are asked to complete a New Patient Packet and fax or email back to our office prior to their first visit. This helps streamline your visit so we can focus on providing quality care.

Welcome to Ponce Primary Care, a full-service primary care practice offering comprehensive prevention and disease management care to adult and teenage patients.

What is Brooke a nurse?

Brooke’s medical interests include pediatrics, women’s health, chronic disease management, and preventative medicine. Having been a registered nurse for many years on a cardiac intensive care unit she also has a strong passion for managing cardiovascular disease.

What is a nurse practitioner?

Nurse Practitioners are highly trained medical providers who are qualified to take care of the full scope of care we offer. Nurse practitioners can evaluate and diagnosis as well as prescribe medication and order imaging if needed. You will receive the same quality of medical care from all of our providers at Ponce Primary Care.

Why do we have open appointments?

We hold open appointments every day so that we can see our patients with urgent medical needs. Prevention-focused visits are by definition not urgent, so we ask patients to schedule them in advance (typically a couple of months). This allows us to manage our appointment slots so that when you are sick we can see you in a timely manner.

Can you do virtual visits in telemedicine?

We now offer “ virtual visits” (i.e. telemedicine) visits in appropriate circumstances. This is an option only for established patients whom we know well and only when a physical exam and vital signs can reasonably be expected to be unnecessary.
