You will need to contact your medical provider’s office for any technical issues. But before you do, please check that you’re attempting to access your patient portal from a desktop computer using a Chrome, Microsoft Edge, Mozilla Firefox, or Safari browser. This may solve the problem. If not, your medical provider can supply additional assistance.
To create a portal account, you will need to call or stop by our office and provide us with your e-mail address. If you don’t have an email address, call us and we will assist you with setting up your portal account. Please note that only patients, legal parents and guardians may create a portal account.
Well exam questionnaires may be completed by logging into your portal account and following the link provided in the patient Message. Please note that you must enter your email address exactly as it appears in our chart. If you have a new email address, please contact us.
Occasionally, upon your request, Plateau Pediatrics will upload documents to your portal account; for example school notes or school immunization forms. These documents require Adobe Reader to download. You may obtain adobe reader free here.

Outpatient: Outpatient programs are for those seeking mental rehab or drug rehab, but who also stay at home every night. Services include individual, couples, family and group therapy. Outpatient treatment services address mental health conditions, addiction and co-occurring disorders.
The New Leaf Recovery Center in Cookeville, Tennessee provides residential treatment and detoxification for individuals with addiction and co-occurring disorders. The Medical and Social Detoxification Programs offers a planned regimen that includes 24-hour medically directed evaluation and monitoring. When detoxification is completed, participants can make a smooth transition into the residential treatment program. The program provides a structured treatment program using evidence-based protocols to assist individuals with the first steps toward recovery. Discharge planning includes the development of a comprehensive, community-based recovery and relapse prevention plan.

This can include sober living arrangements like halfway houses, career counseling, and setting a patient up with community programs like Alcoholics Anonymous (AA) or Narcotics Anonymous (NA).
