Turn left at the end of the Exit onto Prospect Ave. Continue straight on Prospect Ave. and cross over Park Rd., Farmington Ave. and Asylum Ave. Turn right onto Albany Ave. Continue straight until you see the McDonald's on the right. The next drive way is the health center (1229 Albany Ave).
