When will carenotify patient information be removed from the portal?

Information currently contained in the CareNotify patient portal will remain available until midnight on December 21, 2021. Users will have until that time to transfer their existing portal histories to MyHealthHome.

Welcome to Family Health Center of Plainfield

Due to the ongoing COVID-19 (or Coronavirus) pandemic, I am offering telehealth appointments via video chat. I am committed to providing the same experience to my patients as if I were in the office. Telehealth is the provision of healthcare remotely by means of telecommunications technology.

Our Office is conveniently located at the southwest corner of 135th St. and Rt. 30 behind McDonald's and Walgreens, which are just south of Walker's Grove Elementary School.

We’re happy to help you with Patient Portal issues and answer any questions you may have. Call the toll-free support line at 1-855-624-2844, Option 2. This number is available Monday through Friday from 8 a.m. to 5 p.m. Eastern Time.

How to grant access to MyHealthHome?

To grant a caregiver, guardian or trusted family member / friend access to your MyHealthHome account, the requestor will first need to create their own MyHealthHome account. Once they have created their own account, they can request access to your account. You then will receive notice of the request along with an auto-generated PIN code to share with the requesting individual. Once the requesting individual has entered the PIN code, the accounts will be linked and the requestor will be granted access to view your health information.
