To schedule an appointment, or to change an existing appointment, please call 602-234-0004 (Phoenix) or 480-257-2735 (Tempe). Please call 24 hours in advance if you need to cancel or change this appointment. There is a $25 charge if you fail to show for a scheduled appointment or cancel with less than 24-hours’ notice.
