How do I self enroll in the Phoebe patient portal?

If you are not currently enrolled please fill out and submit the Self Enrollment Questionnaire below: Please ensure that you enter the correct spelling of your name with the correct Capitalization (i.e., Phoebe Patient). The email used to register for the Phoebe Patient Portal should be the same email that Phoebe has on file in your medical record.

Can I see records from my Phoebe physicians Doctor's office visit?

Phoebe Patient does show recent visits to Phoebe Primary Care offices. To see records from your Phoebe Physicians doctor’s office visit, you must click 'Office Visit Portal' above or simply click here to log-in/sign-up. You may also contact the Phoebe Physicians customer service line at 229-312-5802.

What do I do if I have trouble logging in to Phoebe patient?

If you experience trouble logging in or accessing Phoebe Patient, please call 229-312-5465, Monday – Friday from 8 a.m. – 5 p.m. If you register for Phoebe Patient and forget your password, you can simply click the “Forgot Password?” link below the sign-in box on the sign-in page.

Dr. Troy Skidmore, a board-certified orthopaedic surgeon, is Medical Director of Orthopaedics for Phoebe Physicians.
