A parent or legal guardian must sign the release for a child or mentally incompetent patient, with required documentation. We require at least 24 hours, and not to exceed 30 days, to release medical information, unless it is a medical emergency.
We very seldom have blood types available, as blood types are not routinely done at PMHC unless you have had surgery where a significant blood loss is possible . For babies' blood types, if you gave birth at PMHC, you would have been informed by the nurse or received a card noting the blood type for you to keep with other birth information. If not, you must follow the same requirements for Release of Information, as stated above.

To request a copy of your medical record, please download and complete the Northwell Health Authorization form. You can complete an authorization by following one of the options listed above. If you are a minor or unable to sign an authorization, the signature of an authorized legal guardian is required. Once your authorization is submitted, please allow three business days before requesting a status.
A minor (under 18 years of age): Complete and submit the Northwell Health Authorization form . In many instances a parent or legal guardian may make a request on behalf of a minor. For further guidance please contact the Health Information Management Department at the facility where the patient was treated.
Requests from the patient and/or appropriate patient representative will be processed within 10 days from the date received. In the event the records are maintained offsite, the facility will communicate this within 10 days to the requestor and the request will be processed within 30 days of receipt unless extenuating circumstances exist, in which case you will be contacted by HIM/Medical Records Personnel.
Patients should allow 3 days from submission before calling for status, or 5 days if the request was mailed.
