How do I register as a patient portal proxy?

If you are an adult (Proxy) wanting electronic access to another patient's health care information through a Patient Portal, a Patient Portal Proxy Registration must be filled out and submitted. How often should I visit my Patient Portal?

Visit us daily, weekly, or at least monthly, because we are always updating your information as you visit and we plan to add new features as they become available within our system. Keeping your health care information private and secure is of the utmost importance. Medical Record Numbers will not be provided by email or over the phone. Your Medical Record Number starts with an “M” and looks like M000055555. The entire number (including zeroes) needs to be entered when using it to sign up for Patient Portal on-line.
