My Penn State Health

A patient will use this portal, if you have been hospitalized at Milton S. Hershey Medical Center or St. Joseph Medical Center. Most patients who attend a Penn State Health Medical Group in Dauphin and Cumberland counties will use this portal. Please call your outpatient clinic to verify.

MyChart

Patients of Holy Spirit Medical Center, or any of their outpatient clinics, will use MyChart as their patient portal. View participating clinics.

IQ Health

Patients who live in Berks County, and who have attended a Penn State Health outpatient clinic in this area, will use this portal. View participating clinics.

Welcome to Hancock Health’s patient portal. From here you can log in to your account to easily access your information, view and request appointments, see your health history and test results, send secure messages to your Hancock Physician Network provider, and view your medications online.

HealthConnect Family Accounts

We can create Family Accounts by enrolling a parent, guardian, or spouse as a Portal User, then attaching the children/spouse/senior parents to that account, provided we have a portal proxy form on file.

Your First Appointment

  • Remember to please arrive early so we can complete your initial paperwork.In addition, please remember to bring the following: Current Insurance Card, list of current prescriptions and over the counter medication, your medical and surgical history, recent test results and revelant records, preferred pharmacy name, phone number, and location.

Appointments and Cancellations

  • We pride ourselves in making every effort to accommodate same day appointments. Please call no less than 24 hours in advance of any appointment that you cannot keep, so we can make that time available for other patients. Any cancellations with less than a 24 hour notice will be charged a $50.00 fee. In addition, this same fee will apply to the second and additional cancellations.
