At the hospital patient portal, you can access read-only medical information through our secure server. This includes educational materials, information on your current medications, allergies, immunizations, health issues, and lab test results. All information is pulled directly from your electronic health records.
To enroll in the patient portal, you must receive services at one of our locations. To create an account, you may self-enroll here. Alternatively, you may contact the location where you received care and tell them you would like to make an account. Registration requires an email and a valid ID.
Whether returning home from the hospital, a skilled nursing facility, or a doctor’s visit, we help our patients safely transfer home with confidence and achieve independence in their community. We help make hospital stays shorter and less frequent by providing healthcare in the home.
Home Health services are discussed with the patient and doctor (at the hospital, primary care physician, specialist, etc.)
Pemi-Baker Hospice & Home Health is Medicare certified and works directly with multiple insurance companies. Most insurances, including Medicare, provide these services at no-cost to the patient or family.
March 16, 2020 : PBCH Suspending All Fitness Memberships. In an effort to work with the CDC guidelines and follow their recommendation to limit exposure, PBCH will be suspending ALL membership classes and programs until further notice. Moving forward auto pays will not be collected until the resumption of services.
