Your Engagement

We want you to be active participants in your health care. As our patient you may access My Patient Connect, our patient portal.

Prescription Refills

Prescription refills may be requested through My Patient Connect or by calling our practice. Please provide 24-48-hour notice for all prescription refills whenever possible.

Specialty Care

We maintain close relations with quality physician specialists in the Concord area and we are happy to refer you to a specialist when appropriate. For all specialty care, your primary care provider will work with you to identify the most appropriate physician for your needs.

Emergencies and Urgent Needs

In the event of an emergency, call 911. If you experience a non emergent healthcare need in the evening or on the weekend, call our practice to reach the on-call CHMG physician who can address concerns that cannot wait until regular office hours.

New Patient Forms

If you’re a new patient with General Physician, PC, we encourage you to contact your provider's office and request any forms needed prior to your appointment. This will help speed up the check-in process so you can see your provider in a timely manner.

There is a charge for the completion of medical forms (i.e. disability, FMLA, school forms, etc). Please consult with the office concerning this fee, as it may change from time to time.
