How to create your patient portal?

Additionally, we are unable to provide care or tests for children who are not Pediatric Place patients. Because of these high volumes, we may run out of same-day appointments. On those days we may advise some of our patients to go to Urgent Care if their problem shouldn’t wait for the following day.

New Patient Forms. These forms must be complete prior to your first appointment! You can fax, email or drop off the forms at the office. After we create your patient records, we will call you to set your appointment: Return to Forms to complete any Visit Forms needed for your child’s upcoming appointment.

Where is Dr. John Bates' pediatric practice?

His wife, Mandy, is from Union City, and they returned in 2007 to establish this practice. John and Mandy attended Freed-Hardeman University for their undergraduate work. After that, Dr. John received his medical training at the University of Alabama and later completed his pediatric residency at Duke Children’s Hospital. He and Mandy have seven children of their own, so he has a great deal of experience, both professionally and personally, when it comes to caring for the wellbeing of children at all ages and stages of life.

Since that time, the Obion County clinic has grown to include a certified pediatric nurse practitioner and several specialized team members to serve your family.
