Employment Income - All household check stubs, with dates, for a complete month (pay stubs or signed statement on company letterhead verifying gross income for the current month, if it’s complete, or the previous month).
In order for you to enroll, you will need to speak to a hospital representative. An active email address will be required for this setup. After you've spoken to a hospital representative, you will receive an email like the one shown below:
To Self Enroll in My Care, you will first need to have some personal information ready. Be prepared with your full name, date of birth, and Medical Record Number (MRN). Without the MRN, you will need to contact a hospital representative. These fields will be compared and must match the information in our hospital record.
