Once you have that code: Visit the Patient Portal site. Click on the link “Activate Account” to log in for the first time. Fill in the requested information.

After hours, an on call provider will return your call to assist you with medical advice and treatment. By contacting your primary care team first, you may be able avoid the need to visit urgent care or the emergency room. If you are having a medical emergency, please call 911.
