Please follow these instructions for registering/signing- in to the Patient Portal: 1 After you receive an email invite to register for the patient portal, go to the link listed in the registration invite email sent to you. If you have not received an email invite to the new patient portal, please call the office to request one sent. 2 Complete the registration steps from the link, with the necessary information. You must enter the same personal information listed on your patient profile that we have in office. You will then be required to create a username and password, including security questions. 3 Once you have completed the form in its entirety, and accepted Registration Disclaimer, you will have access to your personal health record at https://myhealthrecord.com, using the username and password you created.
