Get your medical record number (MRN) and activation code by calling the Patient Portal Help Desk at 734-615-0872 (7:30 am - 4 pm).
Patients can get an activation code after their clinic visit or they can request an activation code by completing the online request form. This code will enable you to login and create your own username and password.

Requests for medical records of deceased patients require a letter of authority in addition to your signed request. The letter of authority is given to the executor of a person's estate by the Probate Court upon their death.
