Your patient portal: stay connected to your health! The patient portal is the best way to communicate directly with your provider, see your lab and medical imaging results, and schedule appointments. Enter here to access or create your account. Your email must be on file with the clinic to access the portal.

Who can access online healthcare information at tcrh?

TCRH has partnered with Syntellis to provide this service. Online healthcare information is available to patients 18 years and older and who have been a patient at TCRH on or after January 1, 2014.

How to Enroll

To enroll in the Patient Portal, you must first complete the Patient Access Form . If you would prefer to complete these forms in person, you can do so at your NVRH healthcare provider’s office or at the Medical Records office located on the lower level of the main hospital building.

This new pre-registration feature allows you to update your demographics, photo IDs, and insurance cards in advance of your appointment.

Health Records Online enables you to manage your health anytime using your computer, smartphone or tablet. Connect with your doctors no matter where you are, at home, around town, or out of state.

Can you share your health records with other providers?

No. Only you may elect to share summaries of your care with other healthcare providers. We do not share your personal information and strictly follow our Privacy Policy. You may read that policy online or when you enroll in Health Records Online.
