Check the email address you provided during your registration process for a portal email with a registration link embedded. Or contact the Navigating Care Support Line at 1-800-925-4456 or email: support@navigatingcare.com Once you have received or created your portal account credentials, please use the following link to access your patient portal.

What is the MyChart patient portal?

When will the carenotify patient portal end?

Information currently contained in the CareNotify patient portal will remain available until midnight on December 21, 2021. Users will have until that time to download or print their existing portal histories for their records.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Northwest Healthcare will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.
