The Patient Portal is a centralized and secure online resource where you have 24-hour access for viewing your medical records, downloading new patient forms, request appointments, review lab results, request prescription refills, pay bills and send a secure message to their provider.

How do I get information from the previous patient portal?

Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that. Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Tennova Healthcare - Clarksville will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.
