If you have questions or would like additional information, please call us at 951-696-6013 and our staff will be happy to assist you. To protect our patients’ privacy and health information, we do not accept telephone requests and we do not disclose information over the telephone.
Radiology images and studies require one full business day to prepare. If you are scheduling follow-up appointments and will need these materials, please plan accordingly.
After we have processed your request for medical records, we will mail the information to the address specified on your request form, or you may pick up your copy from our office. Due to the large volume of requests we receive, it may take up to 5 business days to fulfill your request from the date we receive it.
