How to register for new patient portal?

To sign up for Follow My Health, Click Here. Follow My Health Patient Portal will allow you to access your health information, schedule appointments, send notes to your provider and view your medical visits after your first visit with your provider. Will my medical record from the previous patient portal transfer over to Follow My Health?

No, when you register for the new Follow My Health Portal please be aware that none of your medical history from the previous health portal will be transferred over to the Follow My Health Portal. Rest assured your previous medical visits and your medical records from the former portal are still available to you and your healthcare provider.

The portal is completely secure, so you can be confident that your private information is protected. Only you—or an authorized family member or caregiver—can access your information. Request Access for Another Person

I f you would like to assign a family member or someone you have a close relationship with, access (proxy) to your My SummitCare account, complete the My SummitCare Proxy Access Request Form below and return it to the medical records kiosk on the first floor of Summit Pacific’s Wellness Center.

Appointment Duration

Our office makes every effort possible to see patients in a timely manner. Due to the nature of our practice, emergent circumstances, and complex problems, there may be delays. Please plan for up to several hours for your appointment with our clinic.

Copay

Please bring your copay with you to the visit, if applicable. Please note, we are a specialty clinic and select insurance plans may have a specialist copay that is different than your regular office visit copay. Please contact your insurance for further information.

Pay Your Bill

Payment on your bill can be made securely through our Patient Portal or as a Guest (no login required). If you prefer other options; it can be mailed to our office, accepted over the phone with our billing staff at (503) 850-9940, or made in person at our clinic. We accept cash, card, and check.

Insurance

Although we are on many insurance plans, there are instances when we may not be part of your particular plan. Please make sure you contact your insurance company to double-check that we are within your insurance network.

Physician Referrals

If you have received a referral from your primary care physician, please have it faxed directly to our office, (503) 850-6709, or bring it with you at the time of your visit. If you do not know if a referral is required for you to see a specialist, please inquire with your insurance provider before scheduling an appointment.
