Why is some information missing from the portal?

Diagnostic images, such X-rays, MRIs, and CT scans, are not available on the portal, although the interpretive reports are. For help accessing these types of records or any other records you can’t find on your portal, contact Health Information Management .

When will my lab results be available?

St. John's Health posts your test results in your portal within hours of being finalized. This means you may be seeing your results before your provider has reviewed them.

What are the benefits of the new portal?

The new portal provides access to your single electronic medical record for all services received at St. John’s Health and St. John’s Physician Practices (after October 22, 2018). Now all of your information from the hospital and St. John’s Physician Practices are available in one place.

How do I sign up for the new portal?

During the registration process, you will be asked to provide your email address and select a security question and answer. Then we will email you an invitation with a secure link to complete the enrollment.

If you have deposited as a St. John's student or if you are registered for classes at St. John's, please submit your required health forms via the Online Patient Portal. If you are not able to use the Online Patient Portal, please read Required Health and Immunization Forms for information on other ways to submit your required forms.

Who can access your account?

Rest assured your information will always be kept confidential between you and your Provider. However, you can choose to give another person access to your Ascension St. John Patient Portal account. They will be called your proxy. If you are in person at your clinic, you can fill out the required paperwork to add a proxy.

Having issues with login or site accessibility?

What is the patient portal?

The patient portal is a website that allows you a convenient and secure way to view portions of your medical records and in some locations communicate with participating provider’s health care team. Within your portal, you will be able to do the following:

How can I create an account?

At your next visit, tell the staff you would like to have a portal. The staff will request your email address, and may have you complete a consent form. Within 30 minutes, you will receive an invitation with a link to enroll. Just click on the link and follow the instructions to complete your enrollment.

Can I create an account for a family member who has a Dignity Health doctor?

Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a portal with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age.

Who has access to my account?

Some of the information in your portal is provided through your medical health record and is viewable by you and authorized staff of Dignity Health facilities. If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy.

If my email invitation expires or is lost, how can I request a new one?

The enrollment link expires 90 days from the day they are issued. If you are not able to find your invitation or it expired, you may request a new invitation from the staff at the facility that assist with your medical records.

Who do I contact for help?

Questions regarding specific medical records should be directed to your facility’s Health Information Management (HIM) department. General clinical questions should be directed to your primary care provider’s office. Technical assistance is available 24 hours a day, seven days a week, (877) 621-8014.
