How do I request a patient portal account?

If you have not already been given a username and password for portal access, please download and complete the Patient Portal Request Form, and then drop it off at our office. For security reasons, we cannot create portal accounts over the phone. Please do not use this form for emergencies, appointment or refill requests, or medical questions.

How do I get in contact with a pediatrician?

All members have access to a pediatrician 24/7 via telephone, text, video, or house calls. Take comfort in knowing that your child will only see a board-certified physician. If playback doesn't begin shortly, try restarting your device.

POISON CONTROL

This information is available 24 hours a day, 7 days a week to help you with poison emergencies. AND REMEMBER...Call the Poison Control Center first in a poison emergency. Do not wait until the child shows symptoms.
