Northwest Health provides a Patient Portal help you be informed about your care and what you need to do to stay healthy. The Patient Portal is a secure, online home for your inpatient hospital health information.

What is a patient portal?

Patient portals allow you access to different parts of your information with Northwest Eye Surgeons. Login to experience the features of each site. You must have a registered email address and security code in order to create an account on the patient portal.

What happens after a doctor tests your eye?

After tests are performed, your doctor will evaluate you, inform you of your eye condition, and discuss treatment options if available. In many cases, treatment can begin at your initial visit.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Northwest Health will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.
