How do I access the SCL Health app?

Call (877) 544-3880 if you need assistance. Click “Create Account”. Select three (3) Security Questions. You may not use the same question more than once. You will then be taken into your account in the Patient Portal. Once in the portal, click “myChart” in the top right corner. Then, click “Forms” to complete the New Patient paperwork.

MyHealthOne Portal consolidates many common tasks into one secure, easy-to-use online patient portal. It gives you access to most of your medical records on your desktop computer, laptop, tablet or smartphone 24 hours a day.

Request Copies of Your Medical Records

You can obtain a paper copy of your medical records from HealthONE through our records partner CIOX. In order to request your medical record, download and complete a medical records release form, and then send to the address below.

Your medical records request will be filled within 7-10 business days, and we will inform you if we experience an unexpected delay. Records for physician office visits will be handled urgently to ensure continuity of care. *Please note after the request is filled, delivery to your address may vary due to postal delivery times.

Advanced Wound Care Center

The St. Lucie Medical Center Advanced Wound Center is a specialized center dedicated to caring for individuals with wounds that have resisted healing. How long does it take for a pharmacy to refill?

Please contact your pharmacy for refill requests. Your pharmacy will fax us the request and your refills will be processed within 72 hours. You may also request refills on the patient portal.

Know Before You Go

Legacy has recently updated its intake and consent forms and need for all patients to complete them at the time of their next visit. Once completed, these forms will be good for a year, but we wanted to let you know you will be asked to complete it. You may also download, complete, and bring the forms with you to your next appointment.
