We are excited to present Premier’s online Patient Portal as another step towards creating more efficient and effective communication between you and our office. The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office.

Using a secure password, patients can log into the online Patient Portal 24 hours a day, 7 days a week to: Download copies of medication lists and patient medication education literature Update demographic, medical and surgical history and allergy and medication information

How do I access the information in the Premier family portal?

In order to access the information, you will need to create an account by clicking the button below. You may also log into the portal by going to portal.premierfamily.net. When the account is created, you’ll be able to use the internet to perform tasks that used to require phone calls.

Log into FollowMyHealth and from the left menu, go to the Preferences section and select the appropriate option.

What is proxy access?

Proxy access allows access to the health information of another family member or close relation.

Where does FollowMyHealth come from?

Your FollowMyHealth information comes directly from your electronic medical record at your doctor's office. Ask your doctor to correct any inaccurate information at your next office visit. Your health information is reviewed and updated in your electronic medical record each visit.

What can be done within the document viewer?

The printing, faxing, and emailing of secure messages, scanned documents, appointment information and more can be performed within the document viewer.

Do you have to sign a release of information for follow my health?

No, due to the sensitive nature of medical information , each adult must sign and submit a Release of Information request and establish their own FollowMyHealth account.
