The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office.

Using a secure password, patients can log into the online Patient Portal 24 hours a day, 7 days a week to: Download copies of medication lists and patient medication education literature Update demographic, medical and surgical history and allergy and medication information
In order to access the information, you will need to create an account by clicking the button below. You may also log into the portal by going to portal.premierfamily.net. When the account is created, you’ll be able to use the internet to perform tasks that used to require phone calls.
