The Patient Portal is part of your personal electronic health record, and allows patients to conveniently access their health information from the comfort of their home or office. Patient Portal can also be used to communicate with the practice easily and safely.

When you arrive for your appointment, simply have your driver's license and insurance card ready and proceed to any of the kiosks located to your left or right when you enter. Attention Patients

If you are on treatment for an autoimmune disease, with the exception of Rituxan, which has special recommendations to discuss with your physician, we recommend that you get the COVID-19 Booster shot. The booster should be administered NO SOONER than 6 Months after your second original COVID-19 shot.
