Parents/guardians may communicate with FPC providers and staff as you always have until your child reaches age 18. For Adult Patients: Adult patients can grant portal access to an authorized representative such as a caregiver or an adult child who helps to monitor your health care.

For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.
For this reason, we find it necessary to ask our patients in this age group to complete a consent form naming a parent/guardian as their portal authorized representative.
