Go to the Student Health Center Patient Portal and login using your Pepperdine Network ID and password. Select “Immunizations” in the gray navigation bar/menu at the top of the webpage. Select the appropriate COVID-19 Vaccine section (International or US Authorized).

All students accessing any University campus or facility who do not claim a vaccination exemption must upload proof of vaccination by July 31, 2021 to the Pepperdine Student Health Center using the instructions provided below.
Documentation, including exemption forms and medical records, will be kept confidential. Vaccination status will be shared with University officials who have a legitimate need to know this information. This could include faculty or staff in academic or administrative settings, or for contact tracing purposes, etc.

Before entering Pepperdine University, all Seaver undergraduate students must complete their health history form and enter the dates of and document the required vaccinations in their patient portal. Please note that all medical records must be in English.

Employees who wish to claim an exemption (medical, religious, or philosophical) from the COVID-19 Vaccination Policy may do so by completing the Vaccination Exemption Form for faculty and staff.
How can employees claim a medical exemption from the COVID-19 vaccine?
Quarantine for a full seven (7) days after travel regardless of a negative test (s) or a full 10 days if not tested.
If this test is positive, you will need to isolate for 10 days from the date of the positive test. Pepperdine will assist residential students with an isolation location.
