Users can connect to the child’s information from Phoenix Children's, and also access the child’s health information from other health institutions that have partnered with FollowMyHealth.
Immunizations are no longer available in Phoenix Children’s patient portal at this time. To access immunization information visit the Arizona Department of Health Services (ADHS) webpage. Please note this may not be a complete immunization record due to limitations with reporting to ADHS.

Princeton Community Hospital and PCH Bluefield’s greatest priority during the COVID-19 pandemic is to provide the best possible care while protecting the health and well-being of all patients and employees. To help us achieve that objective, we ask that you please see your family doctor for minor medical issues and routine testing so that our ERs may more efficiently treat those with urgent medical needs.
Two (2) approved caretaker/visitors allowed at a time in patient’s room for pediatric inpatients. Two (2) approved visitors are permitted overnight for pediatric inpatients.
All visitors must be age 18 or above. Visitor must wear a mask at all times, even in the patient’s room. They should bring their own mask. If a visitor chooses not to wear a mask or not to comply with all visitation guidelines, they will be asked to leave immediately. Visitor must stay in the patient’s room.
Princeton Community Hospital will pay up to $437.50/month or $5,250 annually to cover the minimum required loan payment for up to five years. The loan must be the original student loan and not one refinanced. RN will be contractually obligated to stay with the organization for a defined period of time in the same job, FTE, and department.
On December 5, 1970, official ribbon-cutting and dedication ceremonies were held for Princeton Community Hospital. On December 20, 1970, 41 patients were transported from the old Princeton Memorial Hospital to the new facility, and PCH opened its doors to the public.
One (1) caregiver/visitor may accompany the patient and remain in the patient’s room while the patient is in surgery.
