What is a patient portal?

With the Patient Portal, patients can review their contact information , enter their health information (i.e. medications, allergies, past medical history), see past appointment notes, see test & lab results and message the practice. Note: You must have Adobe Acrobat Reader on your device to view past notes or test/lab results.

Founded Eastside Dermatology & Skin Care Center and has served as its medical director since 1987. Dr. Parks is board certified in dermatology with clinical interests including cosmetic and surgical dermatology, laser treatments, BOTOX® Cosmetic, and skin cancer surgery.

Dr. Benjamin Bogucki

Dr. Bogucki is board certified in dermatology and is a member of the American College of Mohs Surgery. His clinical interests include Mohs micrographic surgery, procedural dermatology, as well as general dermatology.

Sandra Walker

Licensed managing esthetician who provides skin care services at our Columbus & Hilliard offices. Sandra has extensive experience in Cosmetic Aesthetics. She strives to attain the best results for her patients’ dermatologic skin care concerns and needs.

How to get a medical history form?

Choose the patient portal tab at the top. Choose the “download your MEDICAL HISTORY FORM” option. Print the paperwork. Complete it at home prior to your appointment time. Fax it back to us at 828-263-0203 OR. Arrive 15 minutes early for your appointment and bring the completed paperwork with you.

What is eprescribing in Medicare?

ePrescribing is defined as a physician’s ability to electronically send an accurate, error free and understandable prescription directly to a pharmacy from the point of care. Congress has determined that the ability to electronically send prescriptions is an important element in improving the quality of patient care. ePrescribing greatly reduces medication errors and enhances patient safety. The Medicare Modernization Act (MMA) of 2003 listed standards that have to be included in the ePrescribe program. These include: (1) Formulary and Benefit transactions: Gives the prescriber information about which drugs are covered by the drug benefit plan (2) Medication history transaction: Provides the physician with information about medications the patient is already taking to minimize the number of adverse drug events. (3) Fill status notifications: Allows the prescriber to receive an electronic notice from the pharmacy telling them if the patient’s prescription has been picked up, not picked up or partially filled.
