How to get your patients to use your patient portal?

The Patient Portal provides you with access to your health care information 24 hours a day. You can request appointments, view lab results, check your medications and more, all from your home computer or smartphone. To enroll in the Patient Portal you must be: At least 18 years old. A current Family Health Center patient.

How to contact a pharmacy after hours?

For after hours medical questions or concerns, please contact the on-call provider by calling our answering service on our main line, 303-948-2676. For routine medication refills, we prefer to receive those electronically from your pharmacy or through the portal. Or, please contact your pharmacy directly.

Where are lab results located?

Labs results (and each individual test value) are located under the "CHART". Once in the Chart, along the left hand side you will see a list of options. Select TEST RESULTS to view your labs. These results will not be made available to you until your provider has reviewed and approved them. Labs Results.

The Family Practice Center, PC, Patient Portal provides secure access to portions of your medical record and allows easy and convenient communication with our practice. So that we can provide this service without compromising your private information, no one will have access to your personal portal account except you.

What age do you have to be to disclose medical information?

For Patients Who Are Minors: Due to federal and state confidentiality laws, patients who are between the ages of 14 and 17 are not required to disclose certain types of medical information to parents/legal guardians.

Our patient portal is a secure online platform that gives our patients convenient, 24-hour access to their personal health information, request appointments, request prescription refills, or communicate directly with our nurses and doctors. Our patient portal also saves you time, while providing important support care between visits.

What else can you do through our patient portal?

Our patient portal allows our patients to take control of their personal health care, from anywhere, as long as they have access to the internet! We have made it easier to avoid trips to the clinic, phone calls, etc. We understand that you have a busy life.

What is the Patient Portal?

The Patient Portal uses HTTPS to provide encrypted communication between you and your doctor's office. Access to your portal account is controlled through secure access codes, personal ID's, and passwords. Only you will have access to the login information needed to view your account.
