Where can I see a neurologist at CMC?

PLEASE NOTE: Patients of CMC’s Comprehensive Neurology will now be seen at CMC’s Queen City Medical Associates practice, located at 775 South Main Street, Manchester NH 03102. This is a temporary change to help us deliver care to all patients as safely as possible.

What can I do with the CMC/CpG patient portal?

With CMC/CPG Patient Portal You Can: View your lab and pathology results in the privacy of your own home. (CPG practices will have this available for viewing on August 7, 2017) Request appointments online with your CPG provider. Securely message your CPG provider at a time that fits your schedule.

How do I contact the CMA Patient Portal support number?

Our combined Patient Portal allows patients to electronically access their medical records for both their hospital and physician office encounters at their convenience on any computer, tablet or mobile device that has internet connectivity.

To gain access follow these steps

Download the registration form. You must have Adobe Reader installed on your computer to view the form. To get Adobe Reader click here. Print and complete the Patient Health Portal Informed Consent to enroll for the portal.

Minneapolis Clinic of Neurology is remaining open in order to be available to you for your neurology needs. During the recommended social distancing due to COVID-19, in addition to office visits, we are offering telemedicine appointments by video for patients that would like to be seen from the safety of their home.
