To use the Patient Portal a username and password will be assigned to you during your office visit. You must have a permanent email address that you check consistently. You may submit a request to change your password at any time by clicking on the option "change/reset password" on the log-on page.

Does the patient portal provide any emergency medical services?

Our Patient Portal is designed to help you monitor your personal medical records. It does not provide any emergency medical service. If you have a medical emergency, call 911.
