Patient Portal

If you are a new patient, you can create an online account using our secure patient portal. The portal provides access to our online patient information, forms and more.

Approved Medications

If you are expecting, we have an approved medications list that you may download for your convenience.

Medical Records and Transfers

Medical record requests will be processed within 7 to 15 business days upon a complete signed medical record request received in our office. There is a 97-cent per page cash fee for production of medical records for personal use and transfer of records. Please see a member of the front office for the necessary form.

FMLA Forms

We will be happy to complete Family Medical Leave Act (FMLA) or disability forms. All FMLA and disability forms will be completed within 7-10 business days from the date submitted to our office. We will contact you upon completion. There is a $25 CASH fee for completion of each form.

Make an Appointment

Patients are seen by appointment – on the same day the appointment is made if possible. You can schedule an appointment online, or call (352) 596-8911 during regular business hours.

New Patients

New patients are asked to arrive 15 minutes early to complete paperwork. You can download, print, and complete the forms prior to your appointment. Please bring the completed forms, a list of all medications you are currently taking, recent copies of x-rays, lab results and your insurance card to your appointment.

Billing

We require payment of your co-payment and past-due account balances at the time of service. We accept cash, checks, debit and all major credit cards.

Insurance

We will submit claims to most major insurance carriers. Please bring your insurance card and I.D. with you to every visit so we can ensure that our records are accurate.

Cancellations or Rescheduling

To better serve the needs of all our patients, we ask that you call at least 24 hours in advance to cancel or reschedule appointments. We make every effort to stay on schedule, although emergencies arise.

Refill Requests

We are happy to help with refill requests during regular office hours, so please call us at (352) 263-2600 to let us know what you need.

After-Hours & Emergencies

If you are experiencing a medical emergency, dial 911. However, if you need after-hours advice for a medical concern, please call (352) 263-2600 and our answering service will assist you.
