New Patient Forms

If you are a new patient, please fill out the registration forms by clicking "Access Patient Forms" below prior to your first visit. This ensures that we have all the information necessary to provide you with quality care and treatment. Please bring the completed forms with you to your appointment.

What to Expect At Your First Visit

Aim to arrive at least 15 minutes prior to your appointment and stay in your vehicle.

Where can I get FollowMyHealth?

You can install the free FollowMyHealth app from the iTunes App Store or Google Play store. Once you have the app, you can choose to specify certain notifications be sent via text message to your mobile phone. Text messages will not contain protected health information.

What is proxy access?

Proxy access enables you to view some of your child’s health information in the portal and, in some instances, communicate through the portal with your child’s healthcare team.
