Patient portal is a secure online website that gives you convenient 24 hour access to your personal health information and medical records, called an electronic health record or EHR –from anywhere with an internet connection. Why is using a patient portal important?
The New York Health Care Proxy Law allows you to appoint someone you trust — for example, a family member or close friend – to make health care decisions for you if you lose the ability to make decisions yourself. By appointing a health care agent, you can make sure that health care providers follow your wishes. Information and Health Care Proxy forms are available from the New York State Department of Health.
Visiting hours for patients including maternity patients are from noon to 8:30 p.m. All visitors are requested to stop at the nurses’ station before entering the patient’s room. Parents of newborns on the maternity unit are welcome to visit from 9 a.m. to 10 p.m.

We offer telemedicine appointments, sometime called video visits, to patients at all our primary care sites. Secure, confidential visits may be conducted via smartphone, tablet or desktop computer during our regular weekday office hours.
