MyChart is MultiCare’s secure, web-based patient information portal. You can log in online from the MyChart home page, or use the MyChart mobile app, to review information from your medical record, including: If you don’t already have a MyChart account, you can request an account access code to get started.
There are two ways you can request copies of your medical records: 1. Complete a behavioral health release form and/or a substance use release form or send a signed and dated letter specifying what is to be released and to whom. You can either fax, mail, or scan and email the form to us as noted below:

Pay online through MyChart, our secure patient records portal. You may use your credit card to pay bills through MyChart, as well as request paperless billing options. If you are not a current MyChart user, your bill will include an access code allowing you to easily sign up for an account.
