Do you need a claim number for Mount Carmel?

You will need to obtain a claim number from your employer. Mount Carmel must include this number on the bill that is sent to the insurance company. If you do not have a claim number from your employer, you are responsible for the entire cost of the services you receive.

Does Mount Carmel pay co-pays?

You are expected to pay co-payments required by your health insurance plan at the time of service. Mount Carmel will bill your primary insurance company for you. Do you have to pay a deposit for maternity care?

Patients who wish to schedule elective medical and/or maternity services are required to pay a deposit and to make arrangements for payment of the balance of charges prior to receiving such services. A patient financial representative will contact you to arrange payment for your bill. If a balance remains after services are provided, a bill for the amount due will be mailed to you.

Co-Payments

In order to keep costs down, we expect payment at the time of service. Co-payments will be collected when you arrive, prior to your visit with the physician. We accept cash, credit or debit cards, and checks. If you have a balance, payment must be made within 30 days unless arrangements have been made with the Office Manager in advance.
