With 6 physicians and 13 advanced practitioners, we are here to meet the needs of the whole family. We offer early morning and evening hours Monday through Friday, with same day appointments available. We are open Saturday and Sunday until 12 for same day visits, 52 weeks a year. Did you know that family medicine includes pediatric care?

How do I sign up for patient portal access?

How to schedule a physical for a medication you have not filled in?

If you are requesting a medication you have not filled in over one year, and you have not been seen for an annual physical, please schedule this physical either at MFM Online Scheduling via DocASAP, by requesting an appointment via our portal or by contacting the office. If the medication needs to be filled prior to this visit, please send a portal message to your provider once you have scheduled the visit. This is for your safety.
