Sep 14, 2015 · Our "HealtheLife" patient portal gives you easy, online access 24 hours a day, to portions of your hospital medical records and test results. HealtheLife is free to use, simple to sign-up for and is completely secure. Only you and your care team at the hospital will have access to your private health information.

Enroll Online Enroll online - Patients who are 18 years or older may self-enroll for the patient portal online.
Patient Portal - MercyOne Statewide View your medical records online using our Patient Portals, available 24/7/365. Updates to your records and new test results post to the portal as soon as they are available. Patient Portals should NOT be used during medical emergencies, call 911 instead for immediate attention.
Sign up for patient portal access 1 Enroll online - Patients who are 18 years or older may self-enroll for the patient portal online. Please click the button above to begin the online enrollment process. 2 Enroll in person - Patients may also ask registration staff at Kossuth Regional Health Center to send them an invitation to the portal. Please bring a photo ID to begin the registration process. A valid email address is required to use the patient portal.

Due to government regulations in regards to patient privacy, at age 12 youth have the right to deny portal access to parents and guardians. Parents may continue to have access to their child/dependent’s portal after age 12 if the child signs a Designation of Patient Portal Access Form.

Verify your identity by entering your legal first name, legal last name and date of birth. Verify your email address and set up a username and password. Select three security questions. Be sure to supply answers that are not easy for others to guess or discover.
